Support at Home

Support at Home – Dietetic Services

Supporting healthy ageing, independence & quality of life.

Good nutrition is an important part of staying healthy, strong and independent as we age. Changes in appetite, mobility, health conditions and the ability to shop or prepare meals can all affect nutritional wellbeing.

Our dietitian works with older adults, families and care teams to identify nutrition concerns early and develop practical strategies that support health and independence at home.

Healthy Ageing & Malnutrition Prevention

We support older adults experiencing or at risk of:

  • Unintentional weight loss and malnutrition
  • Poor appetite or reduced food intake
  • Frailty and loss of muscle mass
  • Difficulty meeting energy and protein requirements
  • Reduced capacity to shop, cook or prepare meals
  • Nutritional challenges following illness, surgery or hospitalisation

Support may include food fortification, high-energy and high-protein strategies, oral nutrition supplements, meal planning and practical recommendations tailored to the client's abilities and home environment.

Dysphagia & Modified Diet Support

For clients with swallowing difficulties, adequate nutrition and hydration can become more challenging.

Working alongside Speech Pathologists and the broader healthcare team, we provide nutrition support for clients requiring texture-modified foods or thickened fluids, helping ensure their diet remains nutritionally adequate, enjoyable and appropriate for their swallowing recommendations.

Support may include meal fortification, suitable food choices, modified meal planning and nutrition education for clients, families and carers.

Chronic Disease Management

Personalised nutrition support can help older adults manage chronic health conditions while maintaining nutritional adequacy and quality of life.

We provide dietetic support for conditions including:

  • Diabetes
  • Cardiovascular Disease
  • High Cholesterol
  • Hypertension
  • Kidney Disease
  • Gastrointestinal Conditions
  • Constipation
  • Diverticular Disease
  • Unintentional Weight Changes

When clients have multiple health conditions, we help simplify sometimes-conflicting dietary advice into practical priorities that are easier to follow.

Meal Planning & Nutrition Support

We can also assist with:

  • Personalised meal planning
  • Meal delivery service recommendations
  • Grocery and food selection guidance
  • Oral nutrition supplement recommendations
  • Nutrition education for clients and carers

NDIS plan-managed & self-managed

NDIS Dietetic Services

Nutrition support aligned with everyday life & NDIS goals.

Nutrition challenges can affect a person's health, independence, participation and ability to manage everyday activities.

Our approach considers the participant's disability, functional capacity, living environment, support needs and NDIS goals to develop practical strategies that can be implemented consistently in daily life.

Please note: we are currently an unregistered NDIS provider and are unable to accept NDIA-managed funding.

Disability-Related Nutrition Support

Dietetic support may include:

  • Nutritional adequacy and malnutrition prevention
  • Healthy eating and balanced nutrition
  • Weight management
  • Meal planning and food routines
  • Limited food variety and feeding challenges
  • Gastrointestinal and bowel health
  • Oral nutrition supplement support
  • Enteral/tube-feeding nutrition support where appropriate
  • Nutrition strategies adapted to functional capacity and support needs

Healthy Eating & Weight Management

We provide practical, non-judgemental support to help participants develop healthier eating patterns while considering disability-related barriers.

Rather than focusing only on weight, we may work towards improving food quality, meal structure, portion awareness, cooking skills and sustainable daily routines that support long-term health.

Support Worker & Carer Education

Consistency between the participant, family and support team can make a significant difference.

We provide nutrition education sessions for support workers, carers and SIL teams to help translate dietetic recommendations into everyday practice.

Education may cover meal planning, healthy food choices, portion guidance, nutrition support strategies, mealtime routines, food fortification and implementation of individual nutrition care plans.

Collaborative Care

With participant consent, we can collaborate with:

  • Support Coordinators
  • Plan Managers
  • SIL Teams
  • Speech Pathologists
  • Occupational Therapists
  • Physiotherapists
  • Behaviour Support Practitioners
  • GPs & Specialists

This helps ensure nutrition strategies are practical, coordinated and appropriate within the participant's broader support plan.

Paediatric

Paediatric Nutrition

Supporting healthy growth, positive eating & confident mealtimes.

Every child's relationship with food is different. Feeding difficulties, sensory preferences, limited food variety or nutritional concerns can make mealtimes challenging for both children and families.

At InHome Nutrition, we provide gentle, family-centred nutrition support to help children meet their nutritional needs, develop positive eating experiences and gradually build confidence with food.

ASD, Sensory Sensitivity & Selective Eating

Children with Autism Spectrum Disorder (ASD) or sensory sensitivities may experience strong preferences around the taste, texture, smell, colour, brand or presentation of food. This can sometimes lead to a very limited range of accepted foods and concerns about nutritional adequacy.

Our dietitian can support with:

  • Picky and selective eating
  • Limited food variety or restricted food repertoire
  • Sensory sensitivities around food
  • Strong preferences for specific textures, brands or food presentation
  • Difficulty accepting or trying unfamiliar foods
  • Nutritional deficiencies or concerns related to restricted intake
  • Establishing positive and predictable mealtime routines
  • Improving nutritional adequacy while working with currently accepted foods
  • Gradual food exploration and exposure strategies
  • Practical nutrition education and support for parents and carers

Our approach is gentle, responsive and pressure-free. We work at the child's individual pace, building on familiar and accepted foods while creating opportunities to safely explore new foods and expand dietary variety over time.

Growth & Nutritional Adequacy

We can also support children experiencing:

  • Poor growth or concerns about weight gain
  • Inadequate energy or protein intake
  • Poor appetite
  • Nutrient deficiencies
  • Highly restricted diets
  • Food allergies or intolerances
  • Constipation and gastrointestinal concerns
  • Need for oral nutrition supplements

Where appropriate, we work collaboratively with parents, paediatricians, GPs, Occupational Therapists, Speech Pathologists and other members of the child's healthcare team to provide coordinated support.
